From commentary -Elihu is saying that Job has not been punished as he deserves, but God has been merciful and had pity on Job and did not punish him according to his sin. That says Job must have really sinned greatly, for his affliction was great and if this greatness of the punishment was "not in great extremity" then how great the wickedness of Job.
